Primary Health Care Structure at Community Level: Supports service delivery, engagement and accountability

Ministry department responsible for community health:Community Health Services Section (CHSS), Preventive Health Services Directorate

PUBLIC HEALTH SYSTEM (includes CORE COMMUNITY HEALTH TEAM), ALTERNATIVE DELIVERY CHANNELS, LOCAL GOVERNEMENT, and COMMUNITY

PUBLIC HEALTH SYSTEM: District Health Management Team, Health Center, and Core Community Health Team 
maintain oversight of Alternative Delivery Channels: Private Providers (Health professionals, clinics, drug shops), CHAM Health Facilities, and Non-Facility-Based Nonprofits (Comprised of many NGOs).

District Health Management Teams supervise Health Centers, which in turn refer/supervise Core Community Health Team (comprised of Assistant Environmental
Health Officer (AEHO), Community Health Nurse (CHN), Community Midwife Assistant (CMA) and Senior HSA & HSA and Community
Health Volunteer (CHV))

Senior HSA & HSA supervise the Community Health Volunteer and Assistant Environmental
Health Officer (AEHO), Community Health Nurse (CHN), Community Midwife Assistant (CMA)

Core Community Health Team refer CHAM Health Facilities

Non-Facility-Based Nonprofits reinforce and strengthen the Core Community Health Team


LOCAL GOVERNMENT: Area Development Committee and Village Development Committee

COMMUNITY: Health Center Management Committee engages and provides accountability to the Health Centers; Community Health Action Group and Village Health Committee
identify and engage with the Core Community Health System
