Summary: USAID’s program to help expand and improve the treatment and prevention of infectious disease, especially HIV/AIDS, includes the following components—

  • using proven strategies for behavior change;
  • increasing access to and utilization of high quality condoms;
  • expanding access to and utilization of quality treatment for sexually-transmitted diseases (STDs);
  • strengthening the capacity of NGOs to respond to the HIV/AIDS epidemic;
  • public-private sector partnership programs for high-risk groups;
  • strengthening the private sector to improve access to and use of quality health products and services;
  • helping the Government of India to implement efforts to eradicate polio;
  • providing technical assistance for tuberculosis (TB) treatment, antimicrobial resistance and other vaccine preventable diseases; and
  • improving surveillance of infectious diseases.

Inputs, Outputs, and Activities: USAID began to tackle the HIV/AIDS issue in India in 1992 by developing the AIDS Prevention and Control (APAC) activity in the south Indian state of Tamil Nadu, one of India's three recognized HIV epicenters. In FY 1999, USAID expanded its program to include the state of Maharashtra, where antenatal HIV prevalence approaches 2%. The seven year, $41.5 million AVERT activity funds comprehensive prevention and care programs throughout the state of Maharashtra. It particularly addresses issues of women and children affected by AIDS. The Government of India (GOI) has requested USAID to consider an extension and expansion of the APAC activity.

FY 2002 Program: In FY 2002, USAID will develop a proposed five-year extension of APAC, to March 2007, with an additional life-of-activity funding of $15.25 million. Besides increasing the number of grants to NGOs, new issues that will be addressed include care and support for HIV affected persons, voluntary counseling and testing, interventions with industrial workers, and geographic expansion of the APAC activity to include Pondicherry, an adjacent and culturally similar area to the present activity area of Tamil Nadu. The rate of HIV infection in Pondicherry is increasing rapidly, and the HIV/AIDS epidemic in Tamil Nadu and Pondicherry are mutually reinforcing. The new initiatives will provide the GOI with cost-effective models based on cutting edge technologies on HIV/AIDS prevention and control. Other USAID activities encompass tuberculosis (TB) treatment, antimicrobial resistance, eradication of polio and other vaccine-preventable diseases, help for children affected by HIV/AIDS, and improved surveillance of infectious diseases.

Planned FY 2003 Program: USAID plans to use FY 2003 resources to fund a new health strategic objective under the proposed new India Country Strategic Plan for FY 2003-FY 2007. The new health strategic objective is expected to include support for ongoing and expanded HIV/AIDS, tuberculosis, and polio activities.

SUBMISSION OF THIS PROGRAM DATA SHEET CONSTITUTES FORMAL RENOTIFICATION OF USAID’S INTENT TO OBLIGATE FY 2002 RESOURCES FOR THE ACTIVITIES DESCRIBED ABOVE.

Performance and Results: Reports from high-risk groups show condom use in recent sexual encounters has increased from 37% in 1996 to 60% in 2000. After a slow start, an AVERT Society has now been formed and independently registered in Maharashtra to implement activities. A statewide HIV/AIDS prevention/destigmatization campaign has been launched. Efforts are underway to build the capacity of the NGO sector to manage and implement HIV/AIDS activities. By 2002, AVERT will be fully functional.

Polio eradication from urban slums and hard-to-reach populations will be an arduous and expensive task. The cases have declined from 1,126 in 1999 to 193 through December 2001. The number of new polio cases is more than anticipated this year, though the number of districts has decreased drastically. USAID will continue to work with the World Health Organization (WHO) to institutionalize Directly Observed Treatment System (DOTS) therapy for TB control. DOTS data indicates a doubling of treatment success over the last 18 months. Over 100,000 people have been tested for TB in Tamil Nadu. With USAID and World Bank support, more than 50% of Tamil Nadu has been covered by DOTS. More than 1,400 health providers have completed in-depth training at the Tuberculosis Research Center during the life span of the project.

Principal Contractors, Grantees, or Agencies: USAID anticipates the continued involvement of key partners, including the World Health Organization (prime); the United Nations Children's Fund (prime); Rotary International (sub); the Centers for Disease Control (prime); International Clinical Epidemiology Network (prime); Voluntary Health Services (prime); Hindustan Latex Family Planning Promotion Trust; Family Health International (prime); and ICICI Limited (sub).
