PVO New Applicant Registration Process
The online registration module consists of 14 pages-13 information and data pages and 1 authorization page. Not all pages will apply to all applicants, but each page must be reviewed. Each page includes additional instructions and tips.
1. To complete the registration application, you will need to supply
the following:
- a DUNS Number, available from Dun & Bradstreet, Inc.
- an IRS Employer Identification Number (U.S.-based organizations)
- two unique e-mail addresses-one for your executive contact and one for your financial contact
- Auditor's name and contact information
- Financial data from your organization's audited financial statements (Non-U.S.-based organizations submit with amounts converted to U.S. dollars.)
- Current U.S. Government grant or contract award information, including amounts
- Overseas activity areas (education, disaster assistance, etc.)
- Countries of activity, including approximate numbers of personnel engaged in the field
- Names of members of the board of directors
- Short narrative description of activities
2. You will need the following information in portable document format (pdf) for uploading (Non-U.S.-based organizations submit documents and English translations when necessary.):
- Articles of incorporation, including amendments, on state letterhead or with state seal (Non-U.S.-based organizations submit charter or other founding document.)
- By-laws
- Audited financial statements for the most recently ended fiscal year, or the fiscal year that ended not more that 15 months before the application submission date (Non-U.S.-based organizations submit with amounts converted to U.S. dollars.)
- Annual report (If an annual report is not available, an executive statement describing activities may be pasted or keyed into a Web form.)

Begin the registration process by requesting a Username and Password at the PVO New Applicant Registration.
Organizations that are no longer listed in the PVO Registry should apply as new applicants.
