If you are interested in applying for an ASHA grant, please complete the Eligibility Questionnaire below to assist you in determining your eligibility. Upon submission of the questionnaire, you will be instructed as to whether you meet the necessary criteria for an ASHA grant.

2. Is the institution you are sponsoring located outside the U.S.?
3. Is the overseas institution private and indigenous (i.e. owned by the citizens of that country in which it is located)?
4. If you represent an educational entity, is it an institution of secondary or higher learning?

5. If you represent a hospital, is it involved in medical education and/or research?

6. Are there a significant number of U.S. citizens and/or U.S. trained personnel at the school or hospital center?
7. Are a majority of your patients or students from the U.S.?
8. Is the institution open to all persons regardless of race, religion, sex, color or national origin?
9. As the sponsoring organization, do you have the title or a long-term lease of at least 50 years on the property of the overseas institution?
10. If you answered "No" to question #9, does the overseas institution have possession of the title?
