PEPFAR and TB: Reduced Impact of HIV/AIDS and TB in Selected
States (briefers)

Problem Statement: HIV/AIDS is a growing menace
in Nigeria . About 2.9 million Nigerians are HIV positive and about
300,000 new infections occur every year. The 20-24 age cohort has
the highest prevalence at 6.5%. Approximately one million Nigerians
have TB/HIV co-infection and about 50,000 new co-infections are
added each year. 1.8 million orphans have already been created by
the disease and this may reach 3 million by the end of the decade.
Stigma, fear, and discrimination against people living with HIV/AIDS
are high. Efforts to develop and disseminate behavior change messages
are increasing, but basic prevention, treatment, care, and support
capacity and infrastructure is weak, with service delivery being
largely in the hands of the private and NGO sectors.
PEPFAR and TB Strategy: Nigeria is a priority country
for the U.S. Government which is presently the largest HIV/AIDS
donor in the country. HIV/AIDS funding increased from a modest $2.7
million in FY 1999 to $25.5 million in 2003. With the advent of
President Bush's Global HIV/AIDS Initiative (GHAI) in 2004, the
U.S. government (USG) funding has increased tremendously as follows:
- $26 Million for COP 2004 with the USAID portion
at $13 Million
- $79 Million for COP 2005 with USAID portion at
$48 Million
- $163 Million for COP 2006 with USAID portion
at $68 Million
- $305 Million for COP 2007 with USAID portion
at $137 Million
These increasing resources will allow the USG to
move from 6 focal states in 2004 to nationwide coverage by the end
of the project period. USAID will collaborate with other USG agencies
in Nigeria to implement sustainable care, prevention and treatment
programs. Specifically, USAID will support the Nigerian national
goal to provide universal access to antiretroviral treatment, focus
behavior change efforts with abstinence and be faithful messages
targeted towards youths and condom and other prevention activities
targeting “most at risk populations” (MARPs) such as
prostitutes, truck drivers, uniformed personnel and people living
with HIV/AIDS. USAID will also provide prevention for positives
and treatment for opportunistic infections and STIs, provide care
and support for People living with HIV/AIDS and orphans and vulnerable
children (OVC), create demand for and provide voluntary counseling
and testing services, strengthen HIV/AIDS activities within Nigeria's
uniformed forces, strengthen national efforts in policy and systems
development, support capacity building for indigenous NGOs, CSOs
and FBOs to respond to IV/AIDS in their communities.
USAID will support systems strengthening for the
Government of Nigeria by providing technical assistance to the National
AIDS Control Agency (NACA) which coordinates the nation's multi-sectoral
response, including the Global Fund for AIDS, Tuberculosis and Malaria
award for HIV/AIDS programming of which 3 are USAID Implementing
Partners (FHI, JSI and SFH). USAID will also continue to support
the Nigeria National Response Information Management System (NNRIMS),
the National logistics system, the Federal Ministry of Health's
(FMoH) Blood Transfusion Service (NBTS), HIV/AIDS Department and
the TB program. USAID supported the development of the HIV/AIDS
Emergency Action Plan and the national policy and will continue
to support the development of an enabling environment for HIV/AIDS+TB
programs.
Expected Results from 2004-2008 in collaboration
with other USG agencies:
- Treat 350,000 PLWHA with Antiretroviral Treatment
(ART);
- Prevent 1.1 million new infections; and
- Provide care and support to 1.7 million HIV-affected
individuals, including 1,350,000 on palliative care and 400,000
orphans and vulnerable children.
